Notice of Privacy Practices
Lake Erie Home Health Care LLC. Effective October 16, 2026.
This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
We are required by law to protect the privacy of protected health information ("PHI"): information that identifies you and relates to your health or care. "You" includes a person receiving care when a family member or legal representative acts for them.
Our responsibilities
- Keep your PHI private and secure.
- Give you this notice and follow it.
- Tell you promptly if a breach may have compromised your PHI.
- Not use or share your PHI in other ways without your written permission, which you may withdraw in writing at any time.
How we may use and share PHI
Care: with the people involved in your care, such as caregivers, supervisors, your doctor and your case manager. Payment: with your health plan or program to confirm coverage and bill for services. Operations: to run the agency, including supervision, training and quality review; companies that help us must agree to protect your PHI.
We may also share PHI when the law requires or permits it: reporting suspected abuse, neglect or exploitation; public health; health oversight such as audits and inspections; court orders and legal proceedings; law enforcement as the law allows; preventing a serious threat to health or safety; and workers' compensation. Ohio law may give your information more protection, and where it does we follow the stricter rule.
When we need your written permission
Marketing, any sale of PHI, and uses not described here.
